Work Permit

Massachusetts State Law:

All minors must obtain and have filled out the promise of employment form for 14 through 17-year-olds before a work permit can be issued. See below for required forms.

Page 1: Promise of Employment (To be completed by Employer)
Page 2: Physician certificate of health for 14- to 15-year-olds. Requires Parent and Student
signatures.
Page 3: Sturgis Student Information Form.

During the school year, return all completed Work Permit Forms to Mrs. Todoroff (East) or Ms. Gowans (West) for a permit to be issued.

After June 13, please email the completed paperwork to Rebekah Benedict at: rbenedict@sturgischarterschool.org. Once the permit is prepared, she will email to arrange a time for the student to sign and receive the permit. Please note that the student must sign the permit in front of the school official.
